What to Bring to your Appointment

At your first appointment please be prepared with the following:
Your List: Questions, Concerns or Symptoms
Most people are notoriously bad at remembering what happened yesterday. So make a note when something happens. Write down the details. Write down your questions as they come up. You will definitely have questions between your initial visit and your followup. Make sure you can remember them to ask. You don't need to give this list to me, it is for you.
Payment. We do not bill insurance, and full payment is due at the time of service. We can provide you with a superbill if you would like to request reimbursement directly from your insurance.
Your Completed Forms
The New Patient Intake Form is several pages long. Keeping a Diary is a project. I understand this. The forms we ask you to complete will depend on your gender, age and concerns. Please start early, so that you have time to really answer the questions. If you fax or email the form(s) to me prior to your visit, you will get better service because I will be more familiar with your case. If you complete the form very early and then let it sit around for a week, you will find that you have more to add to your answers. This is the information that yields gold when trying to figure out stubborn problems.

Include brand names and dosages. Anything that you take counts, including birth control pills. If you have a shelf full of supplements that you're not taking, bring them. There's no point in buying more until we have used the suitable supplements from what you already have. I can help you sort out which ones you should be taking now.
Your Calendar
This will help you plan ahead for your followup, and map out your self-care strategies. You can also book online.
Your Camera
If you have a digital camera or one on your phone we can use it for your medical care. Bring it to your visit if you like.
Your Medical Folder
If you haven't got one yet, it's time to put one together. Collect any medical records that you can, and put them in it. Get copies of your labs and imaging, you have a right to. If you have a complicated condition or a long medical history, you may need a 3-ring binder. I will contribute handouts and resources as we go along.
