What to Bring to your Appointment
with Dr Gryder
At your first appointment please be prepared with the following:
Your List: Questions, Concerns or Symptoms
It's hard enough to remember what happened yesterday; remembering last week may be impossible. Start a list, and add to your notes for a few weeks before we meet. Write down questions as they come up. You don't need to give this list to me, it is for you, so you can remember to ask the important things.
Payment. We do not bill insurance, and full payment is due at the time of service. We can provide you with a superbill if you would like to request reimbursement directly from your insurance.
Your Completed Forms
The New Patient Intake Form is several pages long. Please fill it out. It takes some time to complete, and it will tell me a lot about where you are and what you need. You can bring it in person, or better, fax or email the form(s) to me. If you send them early I will already be thinking about your case.
A List of Your Medications and Supplements
Anything that you take counts. Include brand names and dosages.
Your Calendar
This will help you plan ahead for your followup, and map out your self-care strategies.
Your Medical Folder
If you haven't got one yet, I recommend that you put one together. Start collecting your own medical records. You have a right to any and all of your records including lab results and imaging. I will contribute handouts and resources as we go along.
